Transition1Ready to explore possibilities?

Creating a Life after High School is a five-session program designed for students and their family members as they plan for life after high school. Learn about opportunities and find answers to the questions that are on everyone’s minds:

  • Where can I live?
  • What are the opportunities for learning?
  • Where can I work?
  • What can I do for fun?
  • What agencies can help to provide support?

The individuality of each student is valued and encouraged throughout the sessions.

Dates: October 21 & 28; November 4, 18 & 25

At the end of this program, participants may have the opportunity to apply for funds to support their future vision. PLEASE NOTE: Participants must attend all sessions to be eligible to receive funding.

Registration is open to all students in Monroe County NY, ages 15-21, who are living at home and who have established DDRO eligibility. A family member/guardian or family friend must accompany the student to all sessions.

For students receiving traditional OPWDD-funded services, our grant from Region 1 Developmental Disabilities Regional Office covers the cost of your participation.

For students with a Self-Directed budget, OPWDD requires your budget to cover the cost of participation. Please check with your Support Broker if you have questions about utilizing your budget for this program.

Registration is required by October 14.

To register, call 585-224-7399 or 800-650-4967 ext. 399 or email CAL@starbridgeinc.org
