What is transition?
Planning for the future - whether that means further education, employment, community living, or community engagement - can start at any time. Regulations specify that transition planning must be part of the IEP in effect once a student is age 15. At Starbridge it is our philosophy that it is never too early to start getting ready for an active, engaged future. Starbridge offers a wealth of programs and resources to assist youth and their families in identifying opportunities to realize their goals.

The Coordinator will call you back to ask for information about you or the person you are calling about (if you are phoning for someone else) and can connect you with further resources or staff.
Starbridge, along with includeNYC and the Parent Network of Western NY, is part of New York State Transition Partners, which is funded by the Rehabilitative Services Administration of the US Department of Education. Together, we collect and disseminate information and resources on employment and independent living for young people with disabilities to New York State families.
